Report this with the form below and provide as much information as possible about the medicine, the undesirable effect and the reason for use so that we can investigate your report as well as possible. Please also state your date of birth or age (or that of your patient).
You can also pass on a product complaint or defect to us via the contact form below so that we can investigate what is going on and whether something can be improved on our products. In that case, please also provide the batch / charge / LOT number, which can be found on the packaging by the expiry date.
